As with all veteran benefits, your County Veterans Service Office can assist you with requesting issuance or replacement of military service medals, decorations and awards. Please find the contact information for your County by clicking below:

If you choose to request issuance or replacement military service medals yourself, the eVetRecs (http://www.archives.gov/veterans/evetrecs/) is preferred for requesting medals and awards.
If you would rather make your request by mail, you will need to submit a SF180. Please visit http://www.archives.gov/veterans/military-service-records/replacement-medals.html to retrieve a SF180 form and the addresses to submit the requested paperwork to.
Be sure to provide as much information as possible on the form.
